The services are addressed to:

  • Elderly people
  • Chronic sufferers
  • Disabled
  • Formal caregivers (e.g. nurses, nursing assistants, social workers, etc.) and informal caregivers (relatives of patients, domestic helpers, etc.)
  • Local Government Organizations (L.O.A.)
  • Social organizations and Health Structures (e.g. Eldery Center ,Health Center, Elderly Care Units)
  • Civil Society Organizations (CSOs)
  • Companies

Home interventions

health, safety and empowerment of PWDs, the elderly and their careers

By recording the individual’s history, a detailed description of the needs and deficits is made regarding the emotional and mental balance of themselves and their caregivers as well as the difficulties that require ergonomic intervention with the ultimate goal of safety within the residence. The assessment is done by filling in a brief questionnaire that has been drawn up by our team as health professionals and covers a variety of fields, such as anthropometric characteristics, health status, basic needs, mobility, cognitive ability, social relationships, memory, emotional ability, attitude towards in the dying process, mental health and home security. Depending on the findings of each questionnaire and the diversity of each person, we plan the way of intervention that depends directly on the needs of the elderly or the family caregiver.

During the following interventions, care advice is given by the nurse, a session with the psychologist, ergonomic interventions (e.g. placement of adhesive tapes and night lights to avoid accidents). There is the possibility to donate medical equipment or wheelchairs/walkers to people who need help and who meet the socio-economic criteria of the program. Any incident requiring investigation and further assistance is referred to the competent services.

Interventions in the community 
for the empowerment of professionals and the improvement of the quality of life of citizens

Integrated interventions are organized in collaboration with social agencies and organizations, in matters of self-care and care of patients, elderly, disabled and vulnerable population groups through modern programs and equipment (VR and simulation suit).

During the interventions, the following will be possible:

  • Screening for gerontological vulnerability
  • Trainings in care and self-care for the elderly and their caregivers
  • Cognitive and physical empowerment workshops for the elderly and their informal caregivers
  • Trainings for new mothers and children in first aid, home safety, crisis management
  • Simulation exercises in a geriatric uniform to develop empathy
  • Group counseling by a specialized psychologist
  • Group counseling by a gerontologist

At the same time, citizen awareness actions are being carried out with the aim of cultivating empathy and solidarity towards the elderly, the disabled and more broadly vulnerable population groups.

Corporate social responsibility (CSR) 

actions for the development of solidarity actions and a culture of giving

The co-solidarity days refer to private companies and public bodies (CSOs, local government) that want to carry out solidarity actions in the context of corporate social responsibility actions.

 

The purpose of the program is:

 

  • informing and raising awareness among employees regarding everyone’s right of access to health material
  • developing the culture of exchange and reuse
  • collecting health material on the field during the implementation of the program

In particular, visits to the organization’s environment are organized and the CO-SOLIDARITY PLATFORM team is there to inform and sensitize employees to donate health products, strollers, crutches and consumables or parts/spare parts that are no longer needed. The action starts with a short presentation to the workers – participants about the value of offering and exchanging products we don’t need, but also securing resources for the health system. This is how a solidarity and circular economy will be developed. Also, the participants are informed about the CO-SOLIDARITY PLATFORM, the free educational material related to the care of the elderly and the difficulties faced daily by our citizens who belong to vulnerable groups and are unable to meet their needs with their own means.
